Survey design mistakes to avoid
Twelve quiet mistakes that wreck data quality long before analysis starts.
Most survey problems are not analysis problems — they are design problems committed weeks earlier. Leading questions, broken scales, ambiguous wording, and bad ordering quietly contaminate the data before a single response comes in. The twelve mistakes below are the ones that show up most often, in roughly the order they cause damage.
Wording mistakes that bias the answer
Word choice is where most surveys break. Five patterns to strip out before launch:
- Leading questions — "How great was the new feature?" assumes greatness. Rewrite as "How would you rate the new feature?".
- Double-barreled questions — "Was support fast and helpful?" forces one answer to two things. Split into two questions.
- Loaded language — "Do you support our commitment to customer success?" loads the answer with a yes-bias. Strip the values-laden adjectives.
- Jargon and acronyms — write at the reading level your least-technical respondent shares. If half the audience would Google a word, replace it.
- Double negatives — "Do you disagree that the product is not useful?" is a coin flip even when the respondent has a clear opinion. Rewrite in the positive.

Scale mistakes that drift the data
Scales are the engine of quantitative analysis, and small mistakes here compound. The patterns that matter:
- Unbalanced anchors — "Excellent / very good / good / fair / poor" has four positive labels and one negative. Pair the positives and negatives evenly to keep the average from drifting up.
- Inconsistent scales across the survey — switching from a 5-point to a 7-point scale mid-survey forces re-orientation and adds noise. Pick one scale per construct and reuse.
- Unlabeled middle points — endpoint-only labels invite respondents to invent meaning for the middle. Label every point on Likert scales.

Structural mistakes that lose respondents
Three structural problems together account for most mid-survey drop-off:
- Too long — the single largest predictor of completion is length. If your survey takes more than five minutes for a consumer audience or ten for B2B, you have either a length problem or an incentive problem.
- Sensitive questions early — demographic, salary, or sensitive questions in the first third of the survey cause early drop-off. Put them last when sunk cost keeps respondents committed.
- Required everything — making every question mandatory raises completeness for the responses you keep but lowers the number of responses you keep. Require only the questions you cannot analyze without.

Logic and ordering mistakes
The order of questions changes the answers. The mistakes that cause this:
- Specific before general — asking detailed product feedback before overall satisfaction biases the overall score. Ask the broad attitude question first, then the specifics.
- Priming with examples — listing example answers in the question text plants those answers in the respondent's head. Save examples for the answer choices, not the question.

Program-level mistakes that compound
Even a perfectly designed survey can be undermined by program-level decisions. The big ones:
- Surveying only happy customers — sample bias from suppressing detractors looks great in slides and produces no usable insight. Sample your whole population, not just the ones likely to score high.
- Tying compensation to the score without protecting collection — once a team's bonus depends on the number, the survey gets curated, the sample drifts, and the score stops measuring what it claims to measure.
- Collecting and never acting — the fastest way to kill a feedback program is to stop closing the loop. Response rates fall in the next cycle, and the loss is permanent.
